Internship Site Application Form

Use the following form to apply to become an internship site for the Department of Information Studies at UCLA. Please note, this form is NOT for general UCLA internships or internships for unrelated fields or office-work style internships. It is an expectation that all interns will be supervised by an information professional with an MLS / MLIS degree.

If your internship fits these categories, please fill out the following form. After we receive the application, the Internship Coordinator will contact you by telephone to conduct an application interview.
